1) Full name of the husband
2) Full name of the wife
3) Date of marriage
4) Place of marriage
5) Witnesses (two required)
6) Address and contact details of the couple
7) Details of any previous marriages (if applicable)
8) Copy of ID proof (Aadhar Card/PAN Card/Passport etc.) for both husband and wife.
9) Signature of both husband and wife.
